The bankruptcy petition.
List of creditors.
List of assets and liabilities.
List of current income and current expenditures.
Certificate from credit counseling agency.
Certificate from attorney.
If employed, the debtor must provide to the court evidence showing 60 days of income from the employer -- for example, pay stubs.
For Chapter 7: The debtor must provide a statement of intention, which tells the court whether he or she will keep property that serves as collateral or surrender it to creditors.
For Chapter 13: A repayment plan from credit counseling must be filed at the time of the petition or within 15 days of filing.
